HEALTH INSURANCE AND YOUR THERAPY APPOINTMENT
When making your therapy appointments at Aloha Concepts Counseling in Lahaina, HI, Santa Barbara, or Simi Valley, California, knowing your insurance or EAP details are essential. Whether you’re looking for face to face in person sessions, telephonic sessions or Tele-Video online counseling, being familiar with the details of your insurance plan will help you know what costs you’re responsible for paying. We can help you verify what your plan offers.
How Insurance Works at Our Office
As a courtesy to you, we will bill your insurance. However, you are responsible for the deductibles and co-payment amounts as set forth by your benefit plan and for any uncovered benefit charges.
Please verify coverage and obtain authorization before your first visit. You’re also urged to know your co-pay and deductible. We will file electronically an insurance form of your therapy appointments if they reimburse you for out-of-network claims. We accept cash, checks, and credit cards with an added $3 transaction fee for credit cards.
If you are not the subscriber please bring the subscriber’s name, date of birth, address, social security number, subscriber's employer and phone number with you since these are needed for us to file insurance claims.
We Also Work With EAPs
Often your employer may have an Employee Assistance Program (EAP) where no co-payment is necessary. We encourage you to look into this, as it could save you some money. EAP companies we’re contracted with include:

*For EAP coverage you are responsible for obtaining prior authorization for treatment from your Insurance or EAP carrier. Please be sure to call and obtain an authorization number, which you will bring to your first session Please also ask your employer for the NAME of the EAP COMPANY and their Phone Number so I may know which company to file your claim to.
We’re also contracted on a case-by-case basis with mainland EAP services. Just ask for a single-case agreement if your company is not listed here.

If you’re in Hawaii, please check with us regarding your plan. Some Out-of-network plans must be paid in full at the time of the appointment. Your company will reimburse you for the allowed difference. Insurance companies we’re contracted with are:

When choosing Aloha Concepts Counseling and Annie Vance PhD as your source for therapy in Lahaina, HI, we ask that you make an appointment. This allows us to give each of our patients the time and attention they deserve. Call or text us at 808-740-4288 to request your session. Please include your email, name, and preference of time, a brief description of the issue category and the insurance you will be using. Be sure your phone number is unblocked. You will receive a call from us to set up an appointment for in-person or online counseling, and a text reminder will arrive a day or two before.
